A trainer teaches specific knowledge or skills.

A facilitator guides a group process — the group does the work.

A mediator helps resolve conflicts between people.

These three roles are different. A trainer has expert knowledge and teaches it to others. A facilitator does not need to be an expert in the topic — their job is to guide the group process so people learn from each other. A mediator steps in when there is a conflict and helps the people involved find a solution. In youth work, you often switch between these roles.
